The design of study is descriptive cross sectional survey . The tool of study used for measurement of different parameters is from ten tools of medication error developed by proto-care sciences W.H.O manual . Four private and public sector tertiary care hospitals from Islamabad and Rawalpindi were selected and 500 prescriptions were collected from these hospitals and analysed for medication error using WHO tool 03 which helps to identify major sources of errors within the hospitals.
Analysis of the data showed that the medication error in public sector was 12.5% and 6.8% for private sector. Doctor wrote 12% wrong drug-indication, 08% wrong dose, 05% wrong drug -Allergy , 02% wrong dose.
Medication errors conducted among antibiotics were 13%, 07% antihistamine, Cardiovascular medicines 06% , Central nervous system 04%, anti diabetics 04%, Eyes, ears, nose and throat preparations 03%, hormonal medicines 02%.
These studies shall be expanded at National level through Provicial Government, Pharmacy Services, Health Regulatory bodies and ICT.
